Trade Mark Of Quality is the correct label, indicated by the cigar smoking pig.
Plain white labels with a 1or a 2 centered on the appropriate side.
My copy came in a poly lined RCA Victor inner sleeve with an 8 1/2' x 11" title sheet, loose with the record.
No outer sleeve was include. This may have been issued in many different sleeves, but with the same title page.
